Next-day discharge after C-section may be okay: study

(Reuters) - Some women who deliver their babies by cesarean section may be able to check out of the hospital the next day without raising their risk of problems, according to a Malaysian study.

The study, which appeared in the journal Obstetrics & Gynecology, looked at 360 women in Malaysia, who were randomly assigned to go home either one or two days after having a C-section.

Both groups were equally satisfied with their care, and the women who were discharged sooner seemed to have no more problems with breastfeeding or mental well-being.

"Day 1 discharge compared with day 2 discharge after a planned cesarean delivery resulted in equivalent outcomes," wrote lead author Peng Chiong Tan, at the University of Malaya in Kuala Lumpur.

Those results don't mean that hospitals should start discharging women the day after a C-section, but they do suggest that a next-day discharge is something women can talk about with their doctors, researchers said.

In the United States, where C-sections are done in about one-third of births, women typically stay in the hospital for three to four days after the procedure. That compares with about two days for women who deliver vaginally.

In the past, there were concerns about insurers pushing mothers to leave the hospital before they're ready. That led to a 1996 law requiring insurers to pay for a 48-hour hospital stay after a vaginal delivery and a 96-hour stay after a C-section.

Still, the American College of Obstetricians and Gynecologists (ACOG) says a shorter stay after a C-section is an option if the baby is ready to go home, though the mother should meet certain requirements first such as normal blood pressure, no signs of infection and adequate pain control.

At Tan's hospital in Malaysia, women who have a C-section are routinely told to expect just a two-day stay, and some providers there have discharged new mothers the day after.

Tan's team randomly assigned the 360 women having a planned C-section to go home either one or two days after delivering. In the end, 16 percent of the women in the day-after group were not discharged that early, because either they or their babies were having problems.

But when they did go home the day after, there didn't seem to be a greater risk of difficulties. When the women were interviewed two weeks later, 87 percent were happy with their discharge timing.

The same was true for almost 86 percent of women who went home two days after their C-section.

While the findings would likely extend to women in other countries too, these Malaysian women typically went home to a lot of support - often, an extended family network, Tan said.

"Where this support is not available, next-day hospital discharge may not be associated with the same degree of satisfaction, acceptability and good outcome as we have found," she added.

Several elected officials have proposed a legislative solution that would fix such problems in the future, but one group of opponents is holding it at bay.

The?Queens Gazette?reports?that?Assembly member David Weprin and?Rockland County State Senator David Carlucci (D-Clarkstown) plan to introduce a bill requiring many gas stations to have backup power sources. That would keep the pumps humming in the wake of subsequent storms like Sandy.

The bill would affect all gas stations built or substantially renovated in New York state?after July 1, 2013. It would also affect stations within half a mile of interstates and other evacuation routes. The legislation, as written, requires that all such outlets "be capable of operating all fuel pumps, dispensing equipment, life safety systems and payment acceptance equipment using an alternative generated power source" for at least 72 hours.

Next door in New Jersey,?Assemblywoman BettyLou DeCroce (R-26th) is?sponsoring similar legislation.

Both bills seem modeled on a Florida law that passed in 2007, but just as there, some people are fighting back.?And by "some people", we mean gas station owners and their lobbyists.?

According to the report in?Huffington Post, the?Fuel Merchants Association of New Jersey has been particularly aggressive in its attacks on DeCroce's bill. Lobbyist?Eric DeGesero?claims that if passed, it would pose an unfair burden for gas station owners.

Both bills may ultimately pass, but the legislative process will likely be very, very long. In Florida, the battle dragged on until a compromise was reached. In the end, it was decided that gas stations along evacuation routes must be able to?switch?to generator power, but they don't technically have to?own?generators.?

Should station owners be responsible for backup power? If so, should they pay for it, or should oil companies? Or should we let the stations that can install backup systems do so, leaving them to reap the rewards when storms come ashore??

BPAP: PHL BPO industry to support US amid threat of anti ...

Despite speculations that re-elected US President Barack Obama intends to reopen debates over an anti-outsourcing bill in the US Congress, the Business Processing Association of the Philippines (BPAP) on Friday said it would continue supporting American businesses and the fragile US economy.

The Philippine outsourcing industry would continue helping American companies ?? to ensure they are among the most competitive in the world while freeing up resources to create more jobs in the US,? said BPAP president and CEO Benedict Hernandez.

By outsourcing non-core operations to Philippine-based providers, American companies become more profitable and competitive, Hernandez said.
?
"Profitable companies hire more workers, both here and in the United States," he said.
?
The ?Bring Jobs Home Act? failed to pass in the US Senate last July. Its goal was to remove tax incentives from US companies that outsource non-core operations and manufacturing jobs to other countries. If legislated, the law would give US companies a 20-percent tax deduction on costs associated with closing outsourced operations and transferring jobs to the US.

However, numerous studies have shown that outsourcing has little negative impact on job losses and in fact fosters job growth in companies that outsource business processes, according to BPAP.
?Dartmouth?s Tuck School of Business economist Matthew Slaughter, in a study of the hiring practices of 2,500 US multinationals, found that for every job outsourced, nearly two new jobs are created in the US,? Hernandez noted.

The global outsourcing industry is seen generating $220 billion in revenues this year $35 billion in 2009, according the Everest Group.

?Demand for global IT-BPO services is huge and continues to expand at a rapid rate,? Hernandez said.
